  1. Social Security number or  ITIN number
  2. Date of Birth
  3. Driving licence or TVDL number
  4. Date of getting the licence
  5. Address

Once you call the insurance company, you may have to give your insurance number and request for adding your spouse to your auto insurance. Just mention the required details and any other queries. Make sure to ask how your premium is going to change for the addition. Check how you can reduce the premium in case any increase.
